Puppy Questionnaire

 

 

This is the eMac Bulldogs Puppy Inquiry Form, it is NOT an agreement to purchase a dog or puppy. More questions may follow, plus an interview, vet reference check, and a home check - but this will get us started.  This questionnaire will be used to help match a puppy to your lifestyle, your situation, and your expectations, which gives you and the puppy the best possible chance at a permanent placement.

 

 

 

1.  What is your name, phone number and address:

 

2. How did you hear about eMac Bulldogs?

 

3.  Have you owned a Bulldog before?

 

4.  If you have never owned a Bulldog before, have you been able to spend time with the breed to determine if it is right for you?  Why do you want a Bulldog?

 

5.  Do you want a male or female?  Are you willing to take the other sex if your    choice is not available?

 

6.  What is your color preference? If your color preference is not available, will you take another color?

 

7.  Are you seeking a pet or are you looking for a show prospect?

 

8.  If looking for a show prospect, have you ever shown before?  If so, have you ever completed an AKC Championship title?  Are you aware of the expense and time involved with showing a dog?

 

9.  Are you interested in doing any other activities with your puppy? (obedience, therapy work, other)?  If so, what and why?

 

10.  Are you interested in breeding your dog?  If yes, why?  Have you ever bred dogs before?

 

11.  If seeking a pet only, do you have a problem with the puppy being sold on a

spay/neuter contract?

 

12.  Have you owned dogs before?  If so, did you get any of your previous dogs as a puppy?

 

13.  Are you willing to take a puppy obedience training class with your Bulldog?  What types of things will you do to socialize your puppy and get him/her used to other people, dogs, noises, etc?

 

14. What training methods will you use to handle potential problems such as chewing, barking, and house breaking?

 

15. Are there pets in your home now?  If so, please tell us about them.

 

16. Have you had pets in the past? If so, what happened to them?

 

17. What is your main consideration in choosing a dog?  (examples: the breeder,  price, location, availability, color, pedigree, other)

 

18. Do you have a fenced yard?  If so, what type (chain link, wood, etc)?  If you do not, how will you provide safety to the puppy when outside? 

 

19. Is there a pool or lake on your property?  Are you aware that Bulldogs do NOT swim?

 

20. Do you own/rent?  If you rent, does your landlord allow dogs?  If so may we verify this?

 

21. How many adults/kids live in your home?  If you have children, how much involvement will they have with the raising, care and training of this puppy and in what manner will they be involved? Please list the number of children and their ages.

 

22. Who will be responsible for the care of the Bulldog?

 

23. Where will the dog live or spend most of its time?

 

24. How many hours per day will your puppy be home alone? Where will your puppy stay when s/he is alone in the house? What arrangements have you made to let the puppy out during the day, if you are at work?

 

25. Have you spoken to other breeders and are you on a waiting list for any other litter?

 

26.  What is your time frame for bringing home a puppy?

 

27.  Are you aware that even though the Bulldog has short coat, that they do shed?

 

28.  Are you aware that Bulldogs cannot tolerate hot or cold weather?  Do you have air conditioning?

 

29.  In order of importance what are the top three qualities or characteristics you would like in your puppy and why do you like these particular qualities or characteristics?

 

30. Have you heard of feeding dogs a species appropriate raw diet?  If so, have you fed it before & are you willing to continue feeding your puppy a species appropriate diet?  If not, why not?  Would you like more information on rawfeeding?

 

31.  Have you heard of limiting or completely withholding vaccinations and the possible dangers of over vaccinating?

 

32.  Would you be comfortable limiting or completely withholding vaccinations?  If not, why not?  If yes, have you limited or withheld vaccinations before?

 

33.  Please write a little bit about yourself, your family, your lifestyle, your occupation, basically just a short biography.

 

34.  Please list any questions you may have and any additional comments you would like to make.
